• Use your tab key to move from field to field.
• As you tab through the document, be aware that some of the fields have drop down menus.
• This is a read-only document that you will need to save on your computer.
• After printing the completed evaluation and obtaining original signatures, please make the appropriate copies using the color guideline on the bottom of the form.
